Social Worker/Caseworker (Welfare home)

Location: Woodlands/Buangkok/Bukit Batok

Main Duties and Responsibilities

Daily Duties

· Develop individualise care plans for residents.

· Execute the care plan and monitor progress.

· Coordinate care for residents with relevant stakeholder.

Requirements

· Degree in Social Work, Psychology or Counselling, or a Graduate Diploma in Social Work, Psychology or Counselling.

· Willing to comply with the Standard Operating Procedure when discharging duties.

· Willing to perform shift work in accordance with the Duty Roster.

· At least 1 year of relevant experience.
