As a Staff Nurse (Home Care - Palliative Care), you will be responsible for the following duties:

Provide holistic and patient-centred palliative nursing care to patients in their homes.

Conduct nursing assessments and develop individualised care plans based on patients’ needs.

Perform home visits to monitor patients’ conditions, symptoms, and care requirements.

Provide symptom management, medication administration, wound care, and other nursing procedures.

Collaborate with doctors and the multidisciplinary team to coordinate and review patients’ care.

Provide education, counselling, and emotional support to patients and their families or caregivers.

Guide caregivers on home nursing care, medication management, symptom monitoring, and medical equipment use.

Support patients and families with end-of-life care planning and continuity of care.

Maintain accurate clinical documentation and comply with nursing standards and procedures.

Guide and support junior nursing or care staff where required.

Perform other duties and responsibilities as assigned.

Undertake additional responsibilities as assigned by the Senior Nurse Manager.

Requirements

Possess at least a Diploma in Nursing and registered with Singapore Nursing Board.

At least 3 years of working experiences in inpatient and palliative care.

Possess Advanced Diploma is an advantage.
