- Provide direct counselling, emotional support, and psychoeducation
- Act as liaison for patients/clients and their families
- Maintain accurate, comprehensive, and timely documentation of assessments, interventions, care plans
- Actively participate in and contribute to departmental and organizational quality improvement initiatives
Qualifications, Competencies & Experience
Master/Bachelor/Post Graduate Diploma in Social Work
Registered Social Worker with the Singapore Association of Social Workers (SASW)
Minimum 3 years of relevant working experience in Social Work sector and eldercare setting
Family Service Centre (FSC) experience is required
