STATEMENT OF PURPOSE:

To be part of an integrated care team comprising Primary Care Providers, Community Nurses, Wellbeing Coordinators (WBCs), Community Partners and other care providers to provide efficient and effective integrated person-centred health and social care to residents residing in SingHealth region.

The Assistant Director, Health & Social Integration (HSI) leads and oversees key functions and/or units in the department and assists the Head of Department in managing the day-to-day operations. The Assistant Director is responsible for assessing the health and social care needs of the population in their care setting or region and developing plans with the team to meet these needs to achieve Healthier SG outcomes. This includes establishing new and maintaining existing partnerships with SingHealth multi-disciplinary team members, external partners and agencies in the community landscape, ensuring timely and robust service delivery, and strengthening professional competencies of the WBCs in the team/department. He/she will also help to champion and increase awareness of health and social integration and the WBC team across SingHealth.

DUTIES& RESPONSIBILITIES:

Identify the strengths, weaknesses, opportunities, and threats facing Wellbeing Coordinators’ role in the evolving healthcare landscape. Design strategies to mitigate the challenges and develop newly identified opportunities.

Support the HOD to set a strategic vision, make strategic decisions and plans to achieve cluster, institutional and/or departmental targets, and outcomes.

Drive the change management process to help the team with mindset shift and developing new capabilities to support the evolving healthcare landscape.

Identify and establish new strategic partnerships and strengthen existing partnerships with internal stakeholders as well as external agencies / partners in the development and delivery of services. Demonstrate strong understanding and appreciation of the complementary and competing needs, interests and perspectives in the landscape.

As a domain expert, develop and review existing care models and how the department or team operationalise them.

Lead / co-lead interdisciplinary workgroups or committees at institution or cluster levels and influence discussions on patient care, work processes and policies from the practice of a Wellbeing Coordinator.

Create and enhance awareness of Wellbeing Coordinators and the care they provide within SingHealth and outside of SingHealth with partners and other healthcare clusters.

Set care practice and service standards and ensure that the standards are met by the team.

Analyse data and ground feedback to improve the effectiveness of the department or team in delivering the required services. Identify areas of improvement and supervise the team in quality improvement projects and skills development.

Monitor, review and re-distribute resource deployment and case workload among teams to ensure optimal resource utilisation and achieve required outcomes.

Drive or support the HOD with the department’s workplan and budget planning, and accountable for theappropriate utilisation of budget.

Generate monthly statistical reports as required

Support the HOD to drive the growth, development and retention of staff in the department or team.

Track and ensure that all WBCs have undergone required trainings and competency assessments, and assist HOD in directing continuing professional development of WBCs at departmental level

Work with key stakeholders to establish the required competencies for Wellbeing Coordinators to ensure that the department or team can support the population health needs.

Work with key stakeholders to develop and review the trainings and lesson plans required for the department or team to be equipped with the required competencies.

Conducts effective teaching according to lesson plan, including conducting orientation for new hires, where required

Demonstrates motivation for self-development through continuing professional development activity.

Guide the team in management of complex cases, ensuring that the cases are handled appropriately and staff are resourced to manage these cases in an effective and safe manner.

Provides case consultation and guidance to junior WBCs, especially for case escalations and feedback on individual practice of junior WBCs

Support the HOD in developing and implementing care protocols for the care setting and target population.

Conduct periodic audits on care provision, case management and documentation by WBCs to ensure they are in accordance with established policies and procedures.

Conduct case reviews with the department or team to address any gaps or challenges in care provision and to improve care delivery for WBCs.

Assist HOD to ensure high standards of care documentation, self-grooming and communication among WBCs

Ensure department or team maintain the security and confidentiality of residents or patients’ data.

JOB REQUIREMENTS:

a.  Educational/ Professional Qualifications:

Degree with 10 - 12 years’ experience and demonstrated competency Healthcare-related discipline is preferred

b.  Relevant Experience & Skills

Operational experience in and knowledge of health, social and community care sectors

Good verbal and written communication skills

At least 4 years of leadership experience in managing teams and driving projects to successful completion

Direct responsibility for major programme operations, budget or strategic direction

Advanced knowledge or experience with case work, social work principles, and/or relevant techniques and frameworks

Relevant experience and/or accreditation in similar roles (e.g. health coach, social work etc.) is preferred

c.  Qualities

Able to empathize and relate to others while managing cultural sensitivities

Highly experienced in supervision and leadership of team members

Ability to think critically and make decisions in a highly complex environment

Demonstrates high quality case management ski
