As a Care Concierge, Service Quality (Community Settings), you will be responsible for the following duties:

Serve as the main liaison and single point of contact (Care Concierge) for next-of-kin of nursing home residents.

Engage caregivers and residents pre-admission to address concerns and understand specific needs.

Conduct pre-admission counselling covering fees, services, philosophy of care, living conditions, and expectations.

Welcome residents on admission day, conduct orientation, and ensure smooth settling into the new environment.

Maintain active engagement with residents and caregivers throughout their stay, addressing concerns collaboratively with the multidisciplinary care team.

Participate in multidisciplinary care planning discussions by providing insights on residents’ social background, preferences, and habits.

Develop and maintain key touchpoints throughout the resident journey from pre-admission to discharge.

Assist in measuring and monitoring customer satisfaction through focus groups, audits, and surveys.

Support implementation of service recognition programs and events, such as the annual Service Excellence Award.

Provide feedback on service gaps during team meetings and propose initiatives and training programs for continuous improvement.

Requirements

Possess a min Diploma or Degree with a minimum 2 years of work experiences.
