As a Assistant Care Coordinator (Healthcare), you will be responsible for the following duties:

Conduct pre-admission screening and assess patients’ care needs.

Support nurses and healthcare professionals in providing holistic home-based care.

Monitor patients’ conditions and identify concerns requiring escalation.

Develop and update individualised care plans based on patient needs.

Communicate with patients and caregivers regarding care plans and enquiries.

Monitor vital signs remotely using telehealth systems and digital platforms.

Provide basic health education and lifestyle coaching on diet, nutrition and exercise.

Maintain accurate documentation of patient care, progress and treatment plans.

Coordinate with healthcare providers, community resources and vendors to ensure continuity of care.

Participate in quality improvement, risk management projects and other assigned duties.

Requirements

Minimum Diploma in Health Promotion or healthcare-related disciplines.
