Overview
Under the direction of the supervisor within RICE Community Care department, and in collaboration with Alexandra Hospital and relevant NUHS teams, this position supports core functions that allow the daily operations of the department and the Community Team in AH. The role supports Alexandra Hospital's strategic thrust of integrating care across primary care, community, and home settings through the integrated general hospital model.
The position will support and execute clinical operations for community-oriented aspects of AH’s signature programmes. This role also supports events management and execution, daily operations and administrative functions. The position requires a fast-learning individual capable of detailed execution of tasks, a keen eye for continuous process improvement, and effective communication with stakeholders, including patients.
Job Responsibilities
You will be responsible for the following:
Support Community-oriented Clinical Operations and Business Partnership:
Administrative support of departments’ work in development of integrated care models with a focus on transitional, pre-, and post-hospital workflows, to improve patient outcomes
Support data operations to support clinical operations and decision-making
Administrative support of quality improvement and innovations to improve effectiveness and efficiency of the Community team
Project Support
Operationalise projects that enhance care integration with partners in the community.
Provide operational and administrative support to AH teams giving home- and community-centric care.
Admin support for quality improvement, process improvement projects, adopting methodologies and principles such as Agile, GROSS and GROW.
Conduct selected programmes in the community, including mobility exercise programmes and health education programmes.
Support and run occasional community-oriented events.
Support the engagement and collaboration with healthcare partners in the community.
Support Programme Evaluation and Improvement
Perform data collection through administering surveys and data collation for department programmes, where needed.
Organise and manage data to support department work and evaluation.
Support the daily scheduling, charging and billing operations of the Community team, which may include:
Service counselling via phone calls to patients/caregivers suitable for enrolment to AH Transitional Care (AH TC) and relevant programmes, and enrol them.
Documenting service counselling proceedings and outcomes
Appointment scheduling
Capturing of service charges and bill generation and amendment for AH Transitional Care
Managing the reports required for audit purposes
Ensure efficient service delivery and positive patient experience
Providing operational coverage as needed
Support the daily operations of the office, and support the supervisor in related matters, including:
Perform secretariat work for meetings and projects
Supported related office administrative matters including budget, finance, procurement and inventory management
Support the maintaining records, legal contracts and department data
Other relevant work as assigned by the supervisor
Requirements
Diploma or equivalent qualification in any discipline
Diploma and above in relevant healthcare administration, business administration, or related field is an advantage.
Minimum 2 years of experience working in large organisations, particularly in administration, innovation projects or healthcare operations.
Good proficiency in spoken and written English.
Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook, Teams)
Prior experience in using EPIC and SAP will be an advantage
Understands the importance of teamwork and communication with team members
Able to work in a fast-paced environment
Able to work independently
Ability to work weekends or outside office hours on the occasion is required.
Ability to work off-site in the community on the occasion is required.
Possession of these skills and knowledge would be an advantage:
Experience working in a hospital or community care setting
Knowledge of healthcare systems and processes
Familiarity and experience working with primary care, community health, and long-term care sectors
Knowledge of healthcare regulations and policies
Experience with project management methodologies (e.g. Agile)
Proficiency in data analysis and reporting tools (e.g. Excel, SQL, Power BI, Tableau)
Knowledge of healthcare IT systems and electronic health records
Knowledge and experience of healthcare quality improvement methodologies (e.g. Lean)
