Senior Medical Social Worker (Office Hours | Up to $8K) #HTJ

Job Scope:

• Conduct psychosocial and financial assessments
• Provide direct counselling, emotional support, and psychoeducation
• Act as liaison for patients/clients and their families
• Maintain accurate, comprehensive, and timely documentation of assessments, interventions, care plans
• Actively participate in and contribute to departmental and organizational quality improvement initiatives

Qualifications, Competencies & Experience

Master/Bachelor/Post Graduate Diploma in Social Work

Registered Social Worker with the Singapore Association of Social Workers (SASW)

Minimum 3 years of relevant working experience in Social Work sector and eldercare setting

Family Service Centre (FSC) experience is required
